These documents are helpful to have on hand at the bank for your attorney clients:

Enrollment Form for Lawyers and Law Firms -- This form may be used for converting an

existing trust account to an IOLTA account or for opening new IOLTA accounts.

Change of Status Form -- This form may be used when an attorney's status is changing --

i.e. he or she is changing firms, closing an account, etc.

We request remittance reports be prepared in this format. Completed reports can be submitted to the Bar Foundation via fax, e-mail, or mail. Faxes can be sent to (803) 779-6126. The report may be e-mailed to iolta@scbar.org as an attachment. Or, if preferred, you may mail remittance reports to:
