Enrollment Form for Lawyers and Law Firms
This form may be used for converting an existing trust account to an IOLTA account or for opening new IOLTA accounts.

SC Bar Foundation IOLTA Remittance Form
We request remittance reports be prepared in this format. Completed reports can be submitted to the Bar Foundation via fax, e-mail, or mail. Faxes can be sent to (803) 779-6126. The report may be e-mailed to iolta@scbar.org as an attachment. Or, if preferred, you may mail remittance reports to:
